Here is a media release blank form you can use to create a simple media release for your organization or event.  It is very simple to use – all you need to do is complete the form and print it out or save it as a pdf so you can email it to your local media outlets.

To use the media release blank you will need to use a pdf reader, such as Adobe Acrobat Reader, which you can get here for free.

The media release blank in an interactive PDF form.  When you first open it it looks like this:

It is divided up into the following sections:

Headline – this is where you write a grabby title to attract the attention on the media.

Date – this is the date of the release.   Make sure you put the date of any event in the headline.

Under the line is where the information about your message goes.

Most important information – this goes in the top box.  This is the information you MUST get out into the media.   For example, if you are running an event you want people to attend, the details of the event NEED to be here.  If it is a project and you want people involved, your call to action needs to be here.

Other information – this is where you provide any additional but interesting information.

Event location – provide the address, directions, time and date of where/when you want the media to be.

Photo/video opportunities – provide details of the type of photo/video opportunities that the media will have at your event/activity.  Provide details of interesting people they could interview – like participants.

Media contact – who will the media contact about this release.  Name, phone, email contacts.

So all you need to do is delate the prompts which are in [boxes like this] and type your own words.  This the headline example below:
